Pre-Flight FRAT & Safety Reporting

Open to every NCHF pilot โ€” partner, renter, instructor, or backup tour pilot. No login required.

โš ๏ธ Policy: a completed FRAT is required before every NCHF flight. Per the NCHF Safety Management System (14 CFR Part 5), every pilot completes the Flight Risk Assessment Tool below before each departure. No FRAT, no flight. Record the date, your name, and the score in the aircraft/safety record before you launch.

Total: 0 โ€” LOW. Proceed; brief the highest factor.

Thresholds: 0โ€“6 LOW โ€” proceed, brief the highest factor. 7โ€“12 MEDIUM โ€” proceed only with documented mitigation + Safety Manager review. 13+ HIGH โ€” do not fly until factors are reduced; Accountable Executive sign-off for any exception.

Logging writes this FRAT to the shared NCHF Master Ledger. MEDIUM/HIGH flights are flagged there for Safety Manager sign-off. Strongly encouraged before every flight โ€” the tool won't block you, but the policy is to log it.

Confidential safety / hazard report

See a hazard, error, or near-miss? Report it. Reports may be submitted confidentially and without concern of reprisal (NCHF Just Culture policy). No detail is too small.

Submits to info@flyheliadk.com. The Safety Manager logs it in the hazard register, scores the risk, assigns a control, and closes the loop with you.
